Your First Appointment
As a new patient to Hematology & Oncology Consultants of Pennsylvania, P. C. you likely have many questions and concerns. In order to make your first visit as comfortable and effective as possible, we ask that you complete the New Patient Forms in their entirety, and bring them with you to your appointment. You will receive a New Patient Packet with these forms the mail. Additionally, you may print the forms from our site.
It is extremely important that our team has access to your medical records. If you have not already done so, please complete the Authorization of Medical Records Release form as soon as possible and return it to our office or the office of your primary care provider. We will contact you if we need further assistance obtaining your records.
We recommend, if possible, that you bring another person with you to your first appointment. Because of the complexity and attention to detail, this visit may last 1-2 hours.
Please arrive at the office 30 minutes before your scheduled doctor’s visit and remember to bring:
- Photo ID
- Health insurance and prescription cards
- Insurance referral form from your primary care provider if required
- List of medications and supplements (i.e., vitamins, calcium, over-the-counter medications, herbal remedies, etc.) including strength and dosing. For your convenience, you may bring a bag with all of your pill bottles and one of our staff will enter the information.
- Our medical history form so that you are prepared to discuss those questions with your doctor.
We ask that you kindly avoid using perfumes, body sprays or colognes as patients undergoing chemotherapy are especially sensitive to certain smells.
